Finance Request Form (Required for reimbursement)

These steps should always be followed when requesting money from ACT UP but life happens, especially when you do activist work, and you might have to make a purchase without prior approval. When that happens send an email (one email, not separate) to ERT (ert@actupny.com) and the Finance Team (treasurer@actupny.com) immediately and then fill out the Reimbursement Form. They will then discuss internally on if they will approve your request or not.
